ALL PLAYERS MUST:

• Submit a current physical to their head coach before attending practice
• Submit a COPY of birth certificate to their head coach
• Submit a signed GPYFL Parent/Guardian Consent & Pledge Form to your head coach

All tackle players are required to complete the physical prior to participating in their first practice.  Physicals must be dated within 6 months.

Physicals will need to go to the head coach to hold on to through out the season.
